Patient Forms

If you are a new patient to our office, please complete the New Patient Health History form prior to your first visit. Completing your forms in advance helps us prepare for your appointment and allows us to attend to your dental needs more quickly when you arrive.

You may print the forms, complete them at home, and bring them with you to your appointment. If you prefer, you may also send the completed forms to our office ahead of time.

Completed forms can be faxed to 605-332-9560 or emailed to frontdesk@dakotadentalsd.com prior to your appointment.
